FINANCIAL PLANNING FOR YOUR COSMETIC PROCEDURE
We understand plastic surgery can be a significant investment. Before surgery, you should feel confident in your surgeon and financial planning.
COST OF PROCEDURE
At your consultation, Dr. Kavali or Dr. Moak will work carefully with you to determine your surgery goals. After discussing your goals to create your customized surgical plan, a quote will be made for you. This will include your surgeon’s fees, Monarch Plastic Surgery fees, anesthesia fees, operating room fees, implant costs (if applicable), and the cost of required garments. Your pre-operative and post-operative care are included in your surgery fee. It is important to us that you know the total cost before surgery.
PAYMENT OPTIONS
For elective procedures, payment is due in full two weeks before surgery. You may pay with cash, a cashier’s check, a credit/debit card, Care Credit, or Alphaeon.

We accept cashier’s checks as a form of payment.
Credit/ Debit Cards
We accept all major credit cards, including Visa, Mastercard, American Express, and Discover.
CareCredit
We understand paying your entire balance before surgery may not be feasible. CareCredit is a healthcare credit card that may be used to finance your elective cosmetic surgery procedures and provide you with a flexible payment plan. Financing plans we offer with CareCredit include the following:

FINANCIAL POLICIES
ALL PAYMENTS ARE EXPECTED AT THE TIME OF SERVICE
Payment is required at the time of services rendered unless other arrangements have been made in advance. We accept cash, cashiers’ checks, credit/debit cards, and CareCredit.
INSURANCE
Currently, Dr. Kavali and Dr. Moak do not accept any insurance cases. Please review the options above for alternative financing options or contact our office at 404.250.3333.
COSMETIC SURGERY
All cosmetic fees must be paid before service. To schedule surgery, a non-refundable $1,000 scheduling fee reserves your date. The remaining balance must be received in our office two weeks before surgery. If the payment has not been received, surgery may be canceled, and the scheduling fee will not be returned. If you reschedule your surgery within 14 days of your scheduled surgery date, you are subject to a $200 rescheduling fee.
COOLSCULPTING & ULTHERA
There is a $500 deposit to secure your appointment with our Licensed Medical Esthetician for any CoolSculpting or Ulthera treatments. We will collect your remaining balance when you check in for your appointment. You will not receive a deposit refund if you cancel your appointment within 48 hours.
ADDITIONAL INFORMATION
Disability forms: A $25.00 service charge will be charged for those requiring disability form completion. This charge is due before the forms are sent.
Returned check: There will be a $25.00 service charge on any returned checks.
Surgical Consultation Fee: A $100.00 surgical consultation fee will be charged. This fee is collected when you schedule your appointment.
