The position of the eyebrows plays a central role in facial expression. Even subtle changes in brow height can influence whether someone appears alert, tired, stern, or relaxed. Over time, natural aging, gravity, muscle activity, and changes in skin elasticity can cause the brows to descend. For some patients, this creates a heavy or closed-off look that does not reflect how they feel.
While non-surgical treatments such as neuromodulators can soften expression lines, they do not reposition the brow itself. When brow descent becomes structural rather than muscular, a brow lift may be the most effective way to restore balance and openness to the upper face.
Understanding Brow Aging and Facial Balance
The brows sit at the intersection of multiple facial structures, including the forehead, upper eyelids, and eye sockets. Because the brow and upper eyelids work together visually, brow descent can also make the eyelids appear more hooded. In these cases, addressing the brow position can significantly improve the overall appearance of the eyes without altering their natural shape.
Brow Lift vs. Neuromodulators
Neuromodulators such as BOTOX® are commonly used to relax muscles that create forehead lines and frown lines between the eyebrows. In select patients, neuromodulators can produce a mild lifting effect by reducing downward muscle pull.
However, neuromodulators do not physically reposition sagging tissues. As brow descent progresses, increasing neuromodulator doses may reduce movement but can also contribute to a heavier or lower-looking brow. This is one reason some patients feel their injections “stop working” over time.
A brow lift addresses the underlying structural changes by repositioning the brow itself. Rather than relaxing muscles temporarily, surgery restores the brow to a more balanced position, often allowing neuromodulators to be used more conservatively afterward if desired.
How a Brow Lift Works
A brow lift is a surgical procedure that elevates the brows and adjusts the supporting tissues of the forehead. The goal is not to create an exaggerated or surprised look, but to restore the brows to a position that looks natural, rested, and harmonious with the rest of the face.
During the procedure, the surgeon carefully repositions the brow tissues and addresses excess skin or tissue tension as needed. Incisions are planned with precision to remain well-hidden within the hairline or scalp whenever possible.
Brow lift surgery can be performed on its own or combined with other facial procedures, depending on your anatomy and goals. For example, patients with significant eyelid skin redundancy may benefit from pairing a brow lift with upper eyelid surgery for more complete upper-face rejuvenation.

Endoscopic Brow Lift
The endoscopic brow lift is a minimally invasive approach that uses several small incisions hidden within the scalp. Pretrichial Brow Lift
A pretrichial brow lift places the incision at the hairline. This approach can be helpful for patients with higher foreheads, as it allows brow elevation without further raising the hairline. The incision is carefully designed to blend with the natural hairline.
Coronal Brow Lift
The coronal brow lift uses an incision placed behind the hairline across the scalp. This method may be appropriate for patients requiring more extensive correction. While it is less commonly used today, it remains an option in select cases when significant brow repositioning is needed.

Recovery After a Brow Lift

Many individuals are able to return to desk work or light activities within approximately 10 to 14 days. Tightness, scalp sensitivity, or temporary numbness may occur early in recovery and typically improve as healing progresses.
Your surgeon will provide personalized postoperative instructions, including guidance on incision care, activity restrictions, and follow-up appointments. Healing continues gradually, with results becoming more refined as swelling resolves.
Is a Brow Lift Right for You?

Brow lifts are typically performed as part of an overall anti-aging program, but the procedure is also appropriate for younger adults who have a low brow or who already have deep frown lines due to stress or over-activity of muscles. A brow lift elevates the skin above the eyebrows, causing a reduction in forehead lines and heavy appearance of the upper eye region. At Monarch Plastic Surgery and Skin Renewal Center, we offer a traditional brow lift, when appropriate. We also offer endoscopic brow lift technique, which utilizes minimal incisions hidden in the hair. This allows for a refreshed appearance with less time under anesthesia as well as less downtime and recovery.

In addition to deciding whether a brow lift can meet your personal goals, there are several factors to be considered in determining whether you are a good candidate for a brow lift. Good candidates for a brow lift include those who:
- Have sagging or low-set eyebrows
- Have deep horizontal creases across the forehead and between the eyebrows
- Have a furrowed brow
- You are a nonsmoker, or you are willing to stop smoking before your Forehead Lift/Brow Lift
While a brow lift can address a variety of concerns, it does have limitations. It is important to know these limitations and to understand when you may be a candidate for a different type of surgery. A brow lift may not be a good option for you if you:
- Smoke
- Are not at your optimal health
- Do not have sagging skin or your brow is in the appropriate position
The forehead lift procedure involves a few small incisions hidden in the hair on top of the forehead. The forehead skin is then carefully lifted. The skin and muscles that cause the problems are then removed or modified.
Forehead lift surgery is an outpatient procedure performed with general anesthesia in an operating room. After your surgery, you will be placed in supportive dressings. You will spend a short time in the recovery room before a friend or family member takes you home, where you should plan to rest for the remainder of the day. Be sure to arrange for someone to drive you home after surgery and to stay with you at least the first night. We will advise you on pain medication to ease any discomfort, give you detailed post-operative instructions, and schedule a follow-up appointment to check your progress.
It is expected to experience some pain and swelling in the first few days after surgery.
Incisions usually heal between 2 and 3 weeks after surgery. Depending on your type of work, you can generally return to work in a week to two weeks. Initially, you will have some activity restrictions, primarily aimed at controlling blood pressure in the initial weeks following surgery. Still, most people can return to total activity by 4-6 weeks after surgery.
Every patient’s recovery is different. Your recovery may be quicker or take more time than average.
If you are concerned about sagging along the jawline, cheeks, or neck, you might consider a facelift or necklift at the same time as your forehead lift. Signs of aging in the upper and lower eyelids can be addressed with eyelid contouring surgery (blepharoplasty). Skin quality is best addressed with a comprehensive skin care regimen, which can accentuate your facelift and necklift results. Some lines and hollows will not be addressed by a browlift. Often, it makes sense to use fillers to optimize your results. Botox treatments can enhance the results of your browlift as well.
